Alloy’s purpose driven mission is to provide a holistic solution to help women age healthfully. Alloy helps women age healthfully by providing access to health care professionals, education, and a community of women navigating through life’s natural processes. We are Alloy.
Alloy is not a medical group or a health care provider. Alloy provides its users with the ability to obtain a telemedicine consultation provided by independent medical practitioners including, but not limited to, MDI Health Technologies Ltd. (“MDI Health”), an independent medical group with a network of United States based health care providers (each, a “Health Care Provider”). MDI Health (or your own medical provider if you do not use a MDI Health Provider) is responsible for providing you with a Notice of Privacy Practices describing its collection and use of your health information, not Alloy.

- Personal Information
What is Personal Information?
Personal information is information from and about you that may be able to personally identify you. We treat any information that may identify you as personal information. For example, your name and e-mail address are personal information.
What types of Personal Information do we collect?
We may collect a variety of information from or about you or your devices from various sources, as described below.
Registration and Profile Information. If you sign up for an account, we ask for your email and a password.
Medical Intake Forms and Identity Verification. If you choose to complete a medical intake form in order to meet with a Provider to obtain a prescription, we will collect information about the prescription or service you are seeking as well as information that you provide through the form, such as your symptoms, your and your family’s health and medical history, and your birth date, height, and weight. We will also collect your government-issued ID (a driver’s license, passport, state ID, or permanent resident card) so that the Provider can verify your identity before prescribing any medication.
User Portal. If you log in to our user portal and message your doctor for consultation, a third-party vendor collects information you provide through the messages, such as your symptoms and your health and medical history, and your doctor’s responses. When we provide customer support to Health Care Providers, they may provide us access to your messages.
Prescription Information. We receive information about the prescriptions that the Provider has prescribed to you.
Payment and Delivery Information. If you make a payment through our services, a third party service provider that handles payments for us will receive your payment card information. We do not collect or store financial account information, though we may receive transaction identifiers and summary information that does not include credit card or bank account numbers. We also receive your home address for delivery of your purchases. We use Stripe as our payment processor. You can review their privacy practice here: https://stripe.com/privacy.
Communications. When you contact Alloy directly, such as when you contact our Customer Support team, we will receive your contact information and the contents of your message or any attachments you may send to us, as well as any additional information you choose to provide. When we send you emails, we may use embedded pixels or other technologies to track whether you open them and how you interact with them to learn how to deliver a better customer experience and improve our Services.
Usage Information. We automatically receive information about your interactions with our Services, like the pages or other content you view, the searches you conduct, purchases you make, any content you post, and the dates and times of your visits.
Your Content. We receive any information you post to public areas of our Services.
Device Information. We receive information about the device and software you use to access our Services, including internet protocol (IP) address (from which we can determine general location information), web browser type, operating system version, and device identifiers.
Information from Cookies and Other Tracking Technologies. We and third party partners collect information using cookies, pixel tags, and other tracking technologies. Our third party partners, such as analytics and advertising partners, may use these technologies to collect information about your online activities over time and across different services.
Information from Health Care Providers and Pharmacies. We receive your prescription information from your Health Care Provider and shipping status of your prescriptions from pharmacy providers.

How do I access and correct my Personal Information?
You can review and change your Personal Information by logging into our Services and visiting the Account Preferences section of our Services. You may also notify us through the Contact Information below of any changes or errors in any Personal Information we have about you to ensure that it is complete, accurate, and as current as possible or to delete your account. We cannot delete your personal information except by also deleting your account with us. We may also not be able to accommodate your request if we believe it would violate any law or legal requirement or cause the information to be incorrect.

This Privacy Policy applies to all personal uses of our Services globally and you should not use the Services if you do not agree to the Privacy Policy. Our Services are hosted in the United States and intended for visitors located within the United States. If you choose to use the Services from the European Union or other regions of the world with laws governing data collection and use that may differ from U.S. law, then please note that you are transferring your personal information outside of those regions to the United States for storage and processing. This Consumer Health Data Privacy Policy supplements the Alloy Privacy Policy and applies only to personal data defined as “Consumer Health Data” that is subject to the Washington State My Health My Data Act (MHMDA) and the Nevada Health Data Privacy Act (NHDPA) (“U.S. State Health Privacy Laws”).

Alloy Health California Privacy Notice Addendum
This California Privacy Notice Addendum (this “Notice") supplements the information and disclosures contained in our Privacy Policy. It applies to the residents of California and explains how we collect and process information about you as a business and how you can exercise your privacy rights under the California Consumer Privacy Act (the “CCPA”).
Personal Information means information that identifies, relates to, describes, is reasonably capable of being associated with, or could reasonably be linked, directly or indirectly, with a particular consumer or household, or as otherwise defined by the CCPA.

Over the past 12 months, we have disclosed Identifiers, Commercial Information, and Internet or Other Electronic Network Activity Information to Advertising Partners. - Exercising Your California Privacy Rights
Submitting Requests. You can submit access, correction, and deletion requests by submitting your request through sending an email to support@myalloy.com. You can exercise your right to opt out of “sharing” of your Personal Information for targeted advertising by clicking here. If you are visiting our website using a web browser with the Global Privacy Control (“GPC”) signal enabled, we will treat the GPC signal as a request to opt out of “sharing.”
Verification. Requests for access to, deletion, or correction of Personal Information are subject to our ability to verify your identity. We verify your identity by requiring you to provide the email address associated with your Alloy Health account when you submit a request, providing requested information only to that email address, and, where appropriate, by requesting additional personal information associated with your account.
Authorizing an Agent. If you are acting as an authorized agent to make a request to know, delete, or correct on behalf of a California resident, please send an email to support@myalloy.com and attach a written authorization signed by the resident. We may still require the California resident to directly verify their identity and confirm that they have provided you permission to submit the request.
